Please fill out the required information on this form and submit to the Chairman of the Town of Bear Lake. Town contact information can be found on our website in addition to the Utility Right-Of-Way ordinance that contains important information. https://townofbearlake.com/

Permit Conditions: All land owners need to be notified; roads and ditches need to be put back into original condition when work is completed and inspected by a Town official. Any culverts or signs disrupted during the construction need to be replaced before the crew leaves the area. All costs are the responsibility of the utility company or company doing work for the utility.
